| This Therapist/Case Manager position focuses upon early intervention with children aged 0-5 who have been exposed to violence; and providing specific curriculum based parent/caregiver trainings focused on violence prevention for the 0-5 population. The services offered will be responsible for identifying, assessing and responding to the needs of these children and their families. Primary interventions will include mental health, with collaborative linkages to other resources including domestic violence, HeadStart, preschools and other early childhood providers, health care, law enforcement, DCFS and other coordinating councils and organizations concerned with violence prevention and mental health consultation to pre-school aged children.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
1. Assess, refer, and link families to services
2. Provide supportive counseling in home, community, or office (minimum of once a week)
3. Provide advice, and assist clients in meetings concrete needs
4. Assist family and school or childcare providers in assessing and providing for children's needs
5. Assist family with children's needs
6. Provide crisis intervention when necessary to support, assist and link families to services
7. Identify and access community resources
8. Maintain ongoing communication with childcare providers
9. Maintain ongoing communication with other providers
10. Support families and teams in developing skills in advocacy, facilitation, and accessing resources
11. Attend community meetings as needed
12. Participate in agency meetings and in-service trainings.
13. Maintain records in compliance with agency policies and appropriate clinical practice.
14. Other duties as assigned.
